치의학의 많은 영역에서 치과보건의료 전문가 (DHCP)의 윤리 수준, 치료의 표준 및 치료에 요구되는 기준을 지속적으로 업데이트하고 그 업데이트 된 상태를 유지하는 능력에 의해 결과가 결정됩니다. 치의학은 멈춰진 상태로 있지 않습니다; 현재 치과계에 새로이 발견 사항을 인지하고 윤리적으로 수용하면 환자와 치과보건의료 전문가를 질병으로부터 보호할 수 있습니다. 유닛체어의 수관이 그런 영역 중 하나입니다. 새로운 발견과 규정이 나올 때, 따뜻한 통조림캔을 여는 것처럼 본인이 규정을 준수하고 있는지 의구심이 들 수 있습니다. 문제를 발견하고 해결하는 것은 결정적인 순간이 될 수 있습니다.


우리는 무엇을 알고 있는가?

최근 치과계는 치과내의 수질 안전성에 주목하고 있습니다. 덴탈체어 워터라인 시스템 (DUWS)은 오랫동안 고농도의 잠재적 병원성 박테리아의 근원지로 알려져 왔습니다.


핸드피스의 작은 튜빙은 슈도모나스 (Pseudomonas) 종, 레지오넬라 (Legionella) 종 및 비 결핵균 (nontuberculosis mycobacteria)을 포함한 특정 병원성 박테리아의 서식지로 입증되었습니다. 덴탈체어에서 탐지 된 대부분의 미생물은 공공 급수 시스템에서 비롯되며 건강한 환자에게는 위험하지 않습니다.


최근까지는 면역기능이 저하된 환자에서만 감염될 수 있다고 생각했습니다. 하지만 캘리포니아, 애너하임과 조지아, 애틀랜타에서 최소 90명의 건강한 사람에서 관련된 감염이 발생하면서 다른 시각으로 보게 되었습니다. 덴탈유니트 수관 시스템으로부터 직접 질병이 전염된 사례가 최근 보고 되었습니다.


덴탈 유닛 수관에는 바이오 필름에 취약한 요소가 많이 있습니다. 아래 차트에서 볼 수 있습니다.


최근 건강한 어린이에게 발생한 치과 수관에서 질병을 얻은 사례도 치과계와 미디어 분야의 관심을 받게 되었습니다. 치과의료보건 전문가는 덴탈체어워터라인 시스템을 사용하는 장치의 물 분무와 에어로졸 노출이 위험요소일 수 있다고 지적하고 있습니다.


덴탈체어 워러라인 시스템은 병원체의 저장소인 바이오 필름을 보유할 수 있고, 튜브가 작을수록 바이오 필름 형성 가능성이 커집니다. 많은 연구에 따르면 미생물 오염을 통제하기 위해 증거를 기반으로 한 실용적인 방법이 시급히 필요합니다. 원하는 수준의 집략형성단위 (CFU;colony forming unit)는 식수와 같거나 그보다 적은 경우, 비수술적 시술을 할 경우 덴탈체어 워러라인이 식수에 대한 환경 보호국 (EPA) 표준을 충족하거나 초과된 수치의 물을 사용하고 있는지 확인해야 합니다. (500 CFU / mL의 이질성 박테리아). 일부 제조업체는 특정 장치에 대해 200CFU/mL 정도의 낮은 기준을 필요로 합니다. 개별 유닛의 세부 사항을 알고 있어야 합니다.

바이오필름의 보호하에서 미생물은 항생제, 소독제 및 면역 반응에 적응하고 내성이 생겨 치료의 어려움을 증가시킵니다. 단단한 표면 (예 : 파이프 또는 워터 라인)과 액체 사이의 내부공간은 미생물의 부착 및 성장을 위한 완벽한 환경을 제공합니다. 수관 시스템에서의 바이오필름은 매우 복잡하고 부드럽게 헹구어도 제거되지 않으므로 표면과는 "비가역적 관계"으로 간주됩니다.

A practical guide for testing dental unit waterlines : Part 1


In many areas of dentistry, outcomes are determined by the dental health-care professional’s (DHCP) level of ethics, standard of care, and ability to stay up to date.

Dentistry is not static; being aware of current findings and embracing them ethically can protect both the patient and DHCP from disease. Dental unit waterlines are one of these areas. When new findings and regulations come forward, finding out if you are in compliance can be daunting—like opening a can of worms. Finding out you have an issue and addressing it can be a defining moment.


What do we know? Recently, there has been a focus on water safety in dentistry. Dental unit waterline systems (DUWS) have long been known as a harbor for high concentrations of potentially pathogenic bacteria.


The small tubing has proven a haven for specific pathogenic bacteria, including Pseudomonas spp., Legionella spp., and nontuberculosis mycobacteria. Most microorganisms detected in DUWS come from public water supply and do not pose a risk for healthy patients.


Until recently, it was assumed infection was possible only in immunocompromised patients. But infection outbreaks involving at least 90 healthy patients in Anaheim, California, and Atlanta, Georgia, suggest otherwise. There are now documented cases of direct transmission of disease from DUWS.4–10


Dental unit waterlines have many factors that make them more susceptible to biofilm. They are reviewed in the chart below.


Recent cases of disease acquisition from dental waterlines involving healthy children have also brought the topic to the forefront in dentistry and the media. DHCPs may also be put at risk by exposure to water spray and aerosols from devices that use DUWS.


DUWS can harbor biofilm, a reservoir for pathogens. The smaller the tubing, the greater the chance of biofilm formation. Many studies have shown that evidence-based practical methods for controlling microbial contamination are urgently needed. With the desired level of colony-forming units (CFU) being the same or less than drinking water, DHCPs need to verify they are using water that meets or exceeds Environmental Protection Agency (EPA) standards for drinking water (i.e., less than or equal to 500 CFU/mL of heterotrophic water bacteria) for nonsurgical treatment output water. Some manufacturers require as low as 200 CFU/mL for specific units. Know the details of your individual unit.


The impact of biofilm Microorganisms such as bacteria have the ability to survive by creating a sticky adhesive material called biofilm and attaching to surfaces. Biofilm allows the bacteria to adapt to their environment, especially a hostile one (e.g., running water).


Microorganisms in biofilm “have been shown to elicit specific mechanisms for initial attachment to a surface, development of a community structure and ecosystem, and detachment,” according to a 2002 article in Emerging Infectious Diseases. “Biofilm provides an optimal environment for the exchange of genetic material between cells” and communication using other chemical signals.


Under the protection of biofilm, microbes become tolerant and resistant to antibiotics, disinfectants, and human immune responses, increasing difficulty of clinical treatment. The interface between a solid surface (e.g., a pipe or waterline) and a liquid provides the perfect environment for the attachment and growth of microorganisms. Biofilm in the water system is highly complex and not removed by gentle rinsing, and is thus considered “irreversibly associated” with a surface.


Biofilm accumulation has been shown to be concern with DUWS. Small-bore flexible plastic tubing in dental devices can be coated with well-established biofilms.
