A practical guide for testing dental unit waterlines : Part 1

교직원이자 연자로서 저는 치위생 교육 프로그램에서 학생들을 가르치고 임상가에게 보수교육(CE)하는 것을 영광으로 생각했습니다.

저의 주요 관심사는 초음파 스켈러 (UI) 교육에 있으며, 내 직업 목표는 임상가가 계속해서 커리어를 유지하면서 가르치고, 환자를 보며, 임상 수준을 높이는 것입니다.

따라서 저는 수년에 걸쳐 공통 분모가 있음을 알게되었습니다- 임상가는 종종 그 이유도 모른채 다양한 스트록, 팁 적용과 팁 접근 방식을 구사하며 초음파 스켈러를 다루는 스타일이나 기법이 매우 다르다는 것을 알게되었습니다. 저의 가설은 학교 수업 과정에 기본 내용이 부족하다는 것입니다. 교직원들 간의 기구사용 편차를 보정하는 것도 문제이며, 치위생 교육과정에서 이루어지는 임상 시험이 발전하고 있는 기술을 따라가기에 부족할 수 있다는 증거도 있습니다. 이 기사는 학생, 교직원 및 임상가와 관련된 초음파스켈러의 사용에 대한 3부로 구성된 시리즈 중 첫 번째입니다. 초음파 스켈러 사용 지침에 대한 현재의 트렌드를 나타내기도 합니다. 다음 기사는 기구조작 사용 편자 보정과 적절한 팁 선택과 사용으로 초음파 스켈링을 최적화하며 인체 공학적이며 공기 중 감염을 컨트롤 하는 기술을 다룹니다.


기구의 선택

이 주제의 중요한 점은 치과에서 초음파기구 대비 수기구의 현재 활용도입니다. 일부 조사에 따르면 스케일링을 할 때 70 % 이상이 초음파 스켈러를 활용되는 것으로 나타났습니다. 이러한 경우라면 실습 역량 평가 수준과 함께 초음파 스켈러를 올바르게 사용하기 위해  교육 시간을  재조정하는 것을 생각해 보아야 합니다.


대부분의 치위생 교육 프로그램 (97.82 %)은 실습과정에서 초음파 스케일러를 사용하기 위한 특정 기준이 있으며, 잇몸의 전반에 걸쳐 염증이 있는 (전체 잇몸의 31 %)가 넘는 염증소견을 보여야 사용할 수 있다는 기준을 가지고 있다고 합니다.이는 치위생 프로그램이 여전히 스켈링 기구 사용에 대한 전통적인 접근 방식으로 가르치고 있음을 시사합니다. 방대한 양의 과학적 근거들이 치주질환의 원인이 치석이 아닌 바이오필름이라고 말하고 있다는 것을 고려한다면 이제 교육과정을 업데이트하고 평가해보아야 할 때이지 않을까요?


또 다른 연구에서 “초음파 스켈러 사용 기술이 지난 15 년간 상당히 빠르게 발전해 왔다는데에 저자들이 주목하고 있습니다. 결과적으로 치위생 교육 프로그램과 교수진은 현재의 교육 프로그램이 가장 현대적인 치위생 교육을 따라가고 있다고 말하기는 어려울 것입니다.”


비슷한 내용으로, 미국 치주 학회 (AAP)는 유럽 치주 학회와 함께 2017 년 시카고에서 열린 워크숍을 개최했습니다. 환경 및 시스템 위험 요소를 고려한 전향적 연구와 함께 광범위한 연구 분석을 통해 과학적 증거를 조사하는 것이 목적이었습니다. 미국치주학회의 치주질환 분류 시스템은 1999년 이후로 업데이트되지 않았으며, 그 이후로 치주 및 임플란트 주위염에 대해 알고 있는 내용에 상당한 변화가 있었습니다. 새로운 치주질환 분류 시스템은 진단 과정을 심화하여  폭넓은 정보를 제공하며 일반적으로 전신 건강, 위험 요인 및 병인에 관한 내용들을 다루었습니다. 학계는 또한 과학적인 근거를 제시하는 일을 맡고 있습니다.


현재 방법론 대부분의 치위생 프로그램 (89.2 %)은 초음파 스케일링 사용법 교육 전에 수기구 사용법을 먼저 교육합니다. 응답자의 69 %는 두 가지 방법 모두에 동일한 중요도로 적용되었다고 말했습니다.


학생들에게 소개되는 초음파 스켈러의 종류는 다양하지만 평균은 약 5가지 종류입니다. 학생들이 직접 필요에 의해 구매했거나 학교의 실습실에 구비해달라고 요청한 경우입니다. 일반적으로 초음파 스켈러 실습에서 중요하게 다루어진 점은 스트레이트 팁과 스탠다드와 슬림라인 팁으로 이들은 각기 다른 목적으로 사용됩니다.


과학 논문에서는 직선 디자인의 팁보다는 특정 부위와 치주낭 깊이에 따라 팁을 선택적으로 사용하는 것이 침착물 제거에 있어 효과와 효능을 극대화하며 치근부의 손상을 최소화하고 올바른 팁적용을 할 수 있게 한다고 합니다.


앞서 언급한 설문 조사 결과에 따르면 기구사용법이 특정 부위에 바이오필름을 제거하는데 필요한 특화된 기구들이 있다는 이론에 대해 잘 배우지 못했다고 합니다. 치료 종료 시에 잇몸에 염증이 없어진 것과 치석의 성공적인 제거를 비교하여 평가해야 합니다.


초음파 스켈러 팁에 대한 현재 사용법과 사용방법을 보여주는 이 중요한 기사는 이 분야의 추가 연구가 반드시 이루어져야 한다는 것을 점을 강조하고 싶습니다. 그리고 교육과정 중 초음파 스켈러에 사용시간을 늘리고 올바른 사용 방법에 대한 교육이 필요하다는 것을 강조하기 위함입니다.


교육과정 인증을 받기 위한 기준 치의학 교육과정 인증위원회 (CODA)는 정기적으로 치위생 교육 프로그램에 대한 인정 기준을 업데이트하고 개정합니다. 다음의 기준은 초음파 스켈러에 대한 교육 내용 및 학습 경험 향상의 필요성과, 교수진은 보수교육을 통해 기구사용 시의 편차를 보정하는 과정등이 필요하다는 것을 입증할 수 있습니다.


표준 2-9 :“커리큘럼의 기본 실습 교육에 대해서는 치위생 실습 과정에 과학적 원칙에 기반한 정형화된 실습 과정 순서를 포함해야 하며, 이는 교과 과정 전반에 걸쳐 확장되고 임상 경험 및 조정으로 통합합니다.


의도 : 커리큘럼에 포함된 모든 임상 과정에서 역량을 개발할 기회를 보장하려면 충분한 임상 실습 시간이 필요합니다. 임상 치위생에 관한 교육 자료는 교과 과정 전체에 걸쳐 제시되어야 합니다.


표준 2-10 “계획된 임상 실습 시간은 학생들의 임상 역량과 실제 임상에서 적절한 판단을 할 수 있도록 보장해야 합니다. 임상 실습은 전반적인 치위생 교육과정에서 진행되어야 합니다.”


교육 순서 텍사스 대학 보건 과학 센터 휴스턴, Fones 치위생 학교 및 오하이오 주립 대학은 모두 교과 과정 순서를 변경했습니다. 임상 전단계 과정 내용에  초음파 스켈러와 팁 소개에서 끝났던 기존의 교육 방식과는 다르게  어떻게 팁이 움직이는지 그리고 핸드 스켈러 임상 실습에서 배웠던 기초적인 컨셉인 shank, 스켈러 팁의 작업 부위, 적용과 활용 등이 초음파스켈러 교육에도 소개되었습니다. 치위생과에 들어와서 몇 학기가 지나야 초음파 스켈러를 접해볼 수 있었던 기존의 교육환경에서 학생들이 실습 초기부터 치면세마에서 사용되는 모든 기구에 노출되도록 교육과정을 개편했습니다.


초음파 스켈러에 대한 이러한 노출이 늦어지면 팁 적용시 측방압을 무리하게 주는 경향이 있습니다. 학생들은 수기구를 사용하는 것처럼 초음파를 사용하려고합니다. 왜냐하면 그렇게 배웠기 때문입니다. 오하이오 주립대 학교 (Ohio State University)의 전 프로그램 디렉터 인 미셸 카 (Michele Carr), MA, RDH 말하기를 “초음파 스켈러의 기술과 과학적인 근거들이 발전함에 따라 임상전단계에서 초음파 스켈러를 치면세마에 사용되는 첫 번째 기구로 가르치는 것이 합리적이었습니다. 그 이유는 두 가지였다. 인지 학습 이론은 학생들이 배우는 데 도움이 되는 순차적 발달에 의존합니다. 치위생 교육을 하면서, 우리는 기구를 가볍게 쥐는(light grasp) 평가(assessment)에 사용되는 기구 사용법을 먼저 가르칩니다. 초음파 스켈러도 가볍게 터치해야 합니다. 따라서인지적으로는 상대적으로 세게 기구를 잡아야 하는 수기구 사용법을 교육하기 전에 초음파 스켈러 사용법을 가르치는 것이 합리적이었습니다. 또한,  나는 초음파 스켈러가 대부분의 치과 위생사들이 스켈링 할 때 가장 먼저 사용하는 기구라는 것을 알고 있습니다. 그렇다면 왜 먼저 가르치지 않겠습니까?”


질문은 반드시 해야겠습니다. 요즘 치위생 교육에서 초음파 스켈러가 수기구보다 훨씬 늦게 소개 되는 경우 초음파 스켈러의 중요성에 대한 근거를 가지고 가르치고 있을까요? 이 주제에 대한 세 가지 결론은 캐나다 치위생 프로그램 내에서 수행되는 더 많은 실제 임상 구성 요소 및 실험 연구를 활용한 연구에서 도출되었습니다.


치위생과정에서 초음파 스켈러 교육은 새로운 기술의 잠재력을 충분히 활용하지 못하는 전통적인 접근 방식에 뿌리를 두고 있습니다.


현재의 과학적인 연구 결과에 근거한 치위생 초음파 스켈러 교육 커리큘럼의 표준화는 임상 결과를 향상시킬 것입니다.


초음파 스켈러를 가르치는 임상 치위생 교육자를 위한 지속적인 전문성 개발, 임상 목표의 명확화 및 평가 지침은 치위생 교육 프로그램의 초음파 스켈러 사용에 대한 교육과 실제 사용법을 향상시킬 것입니다.


이 시리즈의 다음 기사에서는 기술 개발 및 최적의 임상 결과에 필수적인 특정 기술 및 초음파 스켈러 사용 교육의 기본 요소에 대해 다룰 것입니다.

In this first of a three-part series on ultrasonic instrumentation

As a faculty member and speaker, I have had the honor of teaching in dental hygiene programs and delivering continuing education (CE).

My main focus is on ultrasonic instrumentation (UI) instruction, and my career goal is to elevate the level at which it is taught, practiced, and executed by clinicians as they continue in their careers.

As such, I have recognized a common denominator over the years— clinicians have very different ultrasonic styles or techniques, using various strokes, adaptations, and approaches, and oftentimes they are not sure why.

My hypothesis is that the foundational content in the academic curricula is lacking. Calibration is an issue among faculty, and there is evidence that clinical competency exams may be lacking in progressive measurement of the developing skill.

This article is the first of a three-part series on UI relating to students, faculty, and practicing clinicians. It portrays current US trends in ultrasonic instruction. The following articles will cover instrumentation techniques with an emphasis on calibration, optimizing ultrasonics utilizing the proper inserts, and technology while incorporating proper ergonomics and aerosol control.


Choices in instrumentation An important aspect of this discussion is the current utilization of ultrasonic versus hand instrumentation in the workplace. Some surveys suggest it is utilized for more than 70% of the scaling or debridement procedure.2 If that is the case, it may be important to look at the amount of time that is spent in the curricula on developing this skill, along with the level of competency evaluation.


Most dental hygiene programs (97.82%) cite calculus as a criterion for using the ultrasonic scaler, with 31% indicating inflammation as a criterion for ultrasonic debridement. This suggests that dental hygiene programs are still teaching a traditional approach to instrumentation. Considering the vast amount of evidence implicating biofilm as the etiological factor for periodontal disease rather than calculus, perhaps it’s time to evaluate and update curricula?


In another study, the authors note, “Ultrasonic instrumentation technology and technique have been evolving fairly rapidly over the last 15 years.  As a result, it can be challenging for dental hygiene programs and their faculty to ensure that the most contemporary approaches are being taught and reinforced throughout all elements of educational programming.”


On a similar note, the American Academy of Periodontology (AAP) along with the European Federation of Periodontology hosted a workshop held in Chicago in 2017. The intent was to examine the scientific evidence through extensive research analysis, along with prospective studies, considering environmental and systemic risk factors. The AAP Classification System had not been updated since 1999, and since then, there have been substantial changes in what we know about periodontal and peri-implant diseases. The new system provides much-needed depth and breadth to the diagnosis process and is more in line with the evidence in regard to systemic health, risk factors, and etiology of the disease in general. Academia is also tasked with teaching to the evidence.


Current methodology he majority of programs (89.2%) introduced hand scaling instrumentation prior to ultrasonic scaling instrumentation. Sixty-nine percent of respondents reported that equal emphasis was placed both methods.


The number of inserts introduced to students varies, but the average is approximately five, either purchased by the students or made available in the program’s dispensaries. Traditionally, the point of instruction is focused on the straight inserts, both standard and the slim lines, with minimal depth of instruction on the variables that cause the inserts/tips to perform differently.


Although scientific literature demonstrates that utilization of site-specific inserts maximizes efficacy and efficiency of deposit removal, minimizes root surface damage, and maximizes correct adaptation of the active tip area, consistent, calibrated instruction on inserts/tips other than the straight design is lacking


The results of the aforementioned survey suggest that the teaching of instrumentation is not fully aligned with the current treatment philosophy of periodontal debridement based on the need for thorough removal of biofilm using site-specific inserts. The end point of therapy measured by resolution or absence of inflammation versus the successful removal of calculus to determine skill competency should be evaluated.


This important article demonstrating the current usage and instruction in UI is imperative for further research in this area to determine the need for curricula changes and sequencing options to elevate the students’ experience with UI


Accreditation standards The Commission on Dental Accreditation (CODA) updates and revises the Accreditation Standards for Dental Hygiene Education Programs on a regular basis. The following standards may demonstrate the need to increase the instructional content and learning experiences with UI and the respective need for faculty to obtain CE and calibration in the areas which they teach.


Standard 2-9 states: “The basic clinical education aspect of the curriculum must include a formal course sequence in scientific principles of dental hygiene practice, which extends throughout the curriculum and is coordinated and integrated with clinical experience . . . ”


Intent: Learning experiences and practice time in clinical procedures are necessary to ensure sufficient opportunity to develop competence in all clinical procedures included in the curriculum. Didactic material on clinical dental hygiene should be presented throughout the curriculum.


Standard 2-10 states: “The number of hours of clinical practice scheduled must ensure that students attain clinical competence and develop appropriate judgment. Clinical practice must be distributed throughout the curriculum.”


Instructional sequencing The University of Texas Health Science Center Houston, Fones School of Dental Hygiene, and The Ohio State University have all changed their curricular sequencing. Introducing ultrasonics in the preclinical course content has been incorporated in the above programs from a minimum of showing students the types of ultrasonic units, inserts and how the technology works, to comparing and contrasting the inserts to hand instruments when foundational concepts such as shank, working edges, adaptation, and activation are being introduced. Regardless of whether the foot pedal is activated, the students are exposed to all instruments used in periodontal debridement, and ultrasonics are not merely an afterthought brought in several semesters later.


If this exposure to ultrasonics is delayed, applying lateral pressure has become ingrained. Students attempt to use ultrasonics like hand instruments, because that is what they learned. Michele Carr, MA, RDH, former program director at Ohio State University, adds


“As the technology and evidence in UI advances, it just made sense to teach it as the first debridement instrument in the preclinical course. The reasons were twofold; cognitive learning theory relies on sequential development, which helps students learn. Translating to dental hygiene, in our program we teach assessment instruments first, which requires a light grasp. UI requires a light touch as well; therefore, cognitively, it was reasonable to teach ultrasonics before hand instruments, which require a firm grasp. In addition, anecdotally, I found that the ultrasonic was the first instrument used for debridement by most dental hygienists. So, why not teach it first?”


The question must be asked: Are today’s programs teaching to the evidence regarding the importance of UI if it is introduced much later and far less in-depth than hand instruments? Three conclusions on this subject were derived from research that utilized more hands-on clinical components and experimental studies done within Canadian dental hygiene programs


Dental hygiene UI education tends to be rooted in a traditional approach that underutilizes the full potential of new technology.

Standardization of dental hygiene ultrasonic curricula based on current evidence would improve practice outcomes.


Ongoing professional development for clinical dental hygiene educators in UI, the articulation of clinical objectives, and the use of evaluation rubrics would enhance UI instruction in dental hygiene programs and its clinical use by new graduates.
